Airway first: Keep upright, do not examine the throat or lie flat; anaesthetist and ENT to secure the airway (AHA 2025)

Investigations

Do not examine the throat or send the child for tests before the airway is secured.

First visit

SpO₂ — Continuous.

Only if indicated

Lateral neck X-ray — Only in a stable patient, with airway equipment ready (thumb sign).
Blood culture and epiglottic swab — After the airway is secured.
