Advice and when to refer

Red flags: Pain over 6 h, vomiting, guarding, fever or a missed period — think appendicitis, obstruction, stones or ectopic pregnancy (NICE CKS)

Investigations

First visit

Urine pregnancy test — Women of child-bearing age — exclude ectopic pregnancy.
Urine routine — Stones or infection.

Only if indicated

USG abdomen — Pain over 6 h, tenderness, fever, vomiting or jaundice.
CBC, amylase/lipase, LFT — Severe or persistent pain.
Erect X-ray abdomen — Suspected obstruction or perforation.
